I just read through this tweet thread from the Lead QA on The Outer Worlds. It’s entertaining and relatable.

Here’s the tweet with a great video.

I didn’t want to paste the whole story here, but the gist is that a handful of conditions and a specifically timed event causes an npc to climb a ladder into space and die, even though they are in a place they aren’t supposed to be able to die.
