Collecting information on joysticks and gamepads; testers needed!

Recently I’ve started a project that will standardize input from many different controllers. To this end, I’ve created a very simple Unity Project which will print out all axes and buttons from up to four different controllers at once in a basic GUI. I’ve done all I can with the controllers at my disposal, but I’d definitely like people with other controllers to share their results so I can support as many gamepads or joysticks as possible.

Of particular interest to me are:
3rd party XBox 360 controllers
Wireless XBox 360 controllers
PS3 controllers

Primarily I’d like results from Android devices, and if anyone has an Ouya console I’d love results from that as well! If you’d like to help, please show me a picture of the gamepad along with some clear way of describing which buttons map to which numbers in the GUI, and also the name of the controller as displayed at the top of the screen. The name must be accurate character-for-character, with all spaces, capital letters and punctuation intact. For example, “XBOX 360 For Windows (Controller)”, not “xbox 360 windows”.
