I set up the XR Interaction Toolkit with the Default Input Actions and XR Device Simulator packages following this tutorial. It seems to have worked perfectly, except for one thing - I can’t make the head rotate. The hands rotate as normal (e.g. I can hold left shift + control, and the left hand rotates; however right mouse + control does nothing).
Has anyone else encountered this?

Yes I’m having the same problem. Tried ttweaking mouse scroll settings in the input manager. Add bindings to head rotation input settings. But no dice. Hope somebody can help. Probably a bug there. Any suggessions to resolve it?

I ran in to the same problem today. The issue was that I had my headset plugged in so when I ran the game it was launching Oculus and using the actual headset for the positioning. I got the Device Simulator working by going in to the project settings and disabling the Oculus plugin.
