How can I make a start screen/transitions between levels?

I want to make one of those start up screens in games where it shows the company logo and then a screen with the title of the game and a ‘press any key to start’ sort of thing. How would I go around doing that? Also, how can I smoothly transition between levels? (for example the screen fades out and then fades into the next level)
