Your product pitch - either a GDD (Game Design Document) or a sheet detailing your planned projects for PlayStation platforms. Include all the necessary game title information (game features, release date, and business model) for your application to be accepted. SIE may reject pitches that include graphic depictions of gratuitous violence, torture, mutilation, or terrorization.
In short: Console development is only open to businesses.

Remember, you can build a PC game with game controller support right away. Once you have created a fun PC game with excellent game controller support, then you can ask the console companies for permission to publish.
Also, you cannot build and play games directly on a retail PS4 or PS5. You would need to become a PlayStation Partner and then get a dev kit from Sony. Then you would be able to build to that dev kit for testing and development. I would recommend building for PC and testing using a PS4/PS5 game controller on PC before worrying about setting up a dev kit.
