It looks like it is based on a Voronoi Diagram.
That looks like the relaxed cone relief map on this page. If you read the rest of that thread, you’ll see some references and some people’s attempts to create such a map for use with that shader. The process seems rather complicated, and nobody in that thread appears to have had any success. That’s not to say it’s impossible by any means–I think apple_motion may have supplied enough information to actually get it done–but it’s probably not worth pursuing if you haven’t read and fully understood the paper his shader is based on.
