There’s nothing wrong with that link, survey monkey is pretty standard. Though I have no interest in card games so I didn’t do the survey.
You certainly do need help.
This is not random survey territory. It’s a fair chunk of time and work as you have formatted it.
A better approach would be to put all of these photos up on your games Facebook page, and tell your community you’ll use the card that gets the most likes.
It’s a relatively low effort way to get feedback. Plenty of people are browsing Facebook and hitting like anyway, so you might as well make them work or you.

May I ask you regarding the survey software, please? I have to do the survey but not sure which tool to use. I know there is Monkey survey (which you use), lime, doodle and so on. Which one is good? Did you like monkey survey?
