Quite simply : The occlusion culling is broken. I’ve tried sizes between 1 and 5 metres on the cell size, I’ve tried all 3 types of Occlusion, and it’s the same result, in the same place in the world:

If I move approximately 1 inch to the right, the scene does this:

I’ve got very little in the world marked as an Occluder; just things like buildings and walls. The roads, lampposts, and other minor or ‘low’ detail aren’t Occluders, just Occludees.
I’m all open for suggestions to fix this!
